Program Description
To assist persons with disabilities in acquiring competitive employment suitable to their vocational skills which will enable them to function as independently as possible.

Services Offered
• Vocational Evaluation
• Work Adjustment and paid work experience

• Developmental skills training
• Leisure time skill development
• Remedial education
• Case Management services--counseling, general problem solving assistance, referrals, etc.
• Consultation with referral agency
• Job seeking skill training, on-the-job training
• Placement and Follow-up
• Miscellaneous support services
• Transportation

Program Eligibility Criteria
• 16-65 years of age
• Physical or mental handicap--either unemployed or underemployed.
• Demonstrate desire to seek competitive employment
• Unable to work competitively due to
(a) poor independent living skills,
(b) weak personal or social skills, limited physical or mental tolerances,
(c) inadequate behavior, and/or
(d) limited productivity.

Program Requirements for Referral Completion
• Individual Rehabilitation Plan established

Program Requirements for Admission
• It is the consensus of the Interdisciplinary Team that the consumer is appropriate for Community Employment in the type of work available.

Exit Criteria
• Consumer is placed in a job of his/her choice which meets the criteria for an adequate job match and has stabilized job performance for a period of 90 days.
• Consumer fails to meet minimum expectations as determined by the IDT Team and is referred to a more appropriate program.
• Consumer chooses to leave program.
