Program Description
The Devleopment Training progran provides functional skill training through repetitive learning activities and related support services to limited productive persons with disabilities in order to increase and/or maintain their overall level of functioning.

Services Offered
• Vocational Evaluation
• Work Orientation and Training; Paid work experiences

• Developmental skill training in the following areas:
– Fine and Gross Motor Development
– Grooming and Self-Help Skills (talking, eating)
– Language
– Reading and writing
– Quantitative Skill Development (numbers, time, money)
– Independent Living/Domestic Behavior
– Community Integration/Socialization
– Recreation/Leisure
• Case management services - written rehabilitation plan, counseling referrals, etc.
• Consultation with referral services
• Miscellaneous support services
• Transportation

Program Eligibility Criteria
• 16 years of age through adulthood.
• Ambulatory, with or without wheelchair.
• Behavior or conduct not harmful to self or others.
• Must have at least partial vision.
• Developmentally Disabled or Physically Disabled.

Program Requirements for Referral Completion
• Individual Rehabilitation Plan established

Program Requirements for Admission
• Individual Rehabilitation Plan established.
• DHS 1010.
• Medicaid Waiver Elegible.

Exit Criteria
• Consumer meets entrance criteria for Sheltered, Supported or Competive Placement Program, the IDT Team agrees that the referral is in the best interest of the consumer and there is an opening in the program to which the referral is made (i.e. funding is available).
• By consumer or guardian decision, the consumer chooses to leave program.
• Consumer fails to meet minimum expectations of the IDT Team and a referral is made to a more appropriate program.

Follow-up
• Program Manager conducts follow-up contacts monthly for three months for consumers who exit only this program. Follow-up for consumers who exit all programs is provided by the lead QMRP/Program Manager.
