Program Description
The CILA program provides with developmental disabilities training and support services which allow them to live in the home of their choice and to be active, productive members of their communities.
Services Offered - Housing
• 24 Hour Supervision.
• Personal Care Skill Training
• Daily Living Skill Training
• Socialization/Recreation Skill Training
• Transportation
• Case Management/Case Coordination Support & Linkage
• Vocational Training (Supported Employment/Developmental Training)
• Financial Management Training
Program Eligibility Criteria
• 18 years of Age
• Developmentally Disabled
• Requires an array of services and a supervised living arrangement.
Program Requirements for Referral Completion
• Intake Summary Form
• Proof of Medicaid Waiver eligibility.
• Pre-Assessment Screening (PAS) Referral
Program Requirements for Admission
• Medical examination completed within 12 months preceding admission.
• Dental examination completed within 12 months of admission.
• Psychological Evaluation
• Nusing Packet
• Social Assessment.
• Inventory for Client and Agency Planning (ICAP) or the Scales of Independent Behavior (SIB).
• Educational and Vocational Assessment.
• A communication screening in vision, hearing, speech, language and sign language.
• Others as required by the disability of the individual such as physical therapy, occupational therapy and activity therapy.
• PAS Referral Packet
Exit Criteria
• Medical needs of the individual cannot be met by CILA.
• The behavior of the individual places the individual or others in serious danger.
• The individual is to be transferred to a program offered by another agency and the transfer has been agreed upon by the individual, the individual's guardian, the transferring agency, and the receiving agency.
